PROJECT DESCRIPTION
Working with the Nebraska Department of Health and Human Services on this project, our goal was to take a sort-of mundane subject and bring it to life with the stories of people whose lives have been touched by Community Health Workers.
Community Health Workers (CHWs) are an asset in our healthcare system, yet very few know the depth and value of their involvement. CHWs have a close understanding of the community they serve, building relationships with patients and linking them to local health and social services.
This video of Maria and Belen’s story shows the incredible effect of having CHWs like Maria doing their work each day — her involvement, care and attention has been instrumental in Belen’s health. Our goal was to show the breadth of what services CHWs provide, while ultimately showing the deeply personal effect these patient-CHW relationships have on the health of a whole community.
